A Kentucky Department of Agriculture report, released Friday, says the condition of a cable that snapped and the operator's failure to hit an emergency stop button were the largest factors in the accident.

Kaitlyn Lasitter's legs were severed last June on the Superman Tower of Power Ride at Six Flags Kentucky Kingdom in Louisville. Doctors were able to reattach her right foot, but not her left.

Kentucky Kingdom spokesman Carolyn McLean says the park has audited maintenance, training and safety and is working with the Lasitter family to ensure Kaitlyn is taken care of.
