LUFKIN, Texas - An East Texas dialysis center closed since April following an increase in deaths and life-threatening complications will reopen Wednesday after getting clearance from the state, DaVita Inc. company spokesman Michael Chee said.

A former DaVita nurse has been accused of intentionally injecting two patients with bleach. Kimberly Clark Saenz, 34, of Pollok, has been charged with two counts of aggravated assault. Both patients survived.
A spokesman for Texas Department of State Health Services said in Tuesday's editions of The Lufkin Daily News that the facility will gradually build up the number of patients.
Most details of the center's deficiencies were blacked out in documents obtained by the newspaper through an open records request. The number of patient deaths under review has not been released.
Health department spokesman Doug McBride said the facility will be closely monitored by the state for at least six months.
El Segundo, Calif.-based DaVita has more than 1,300 outpatient facilities across the United States.
