Joan Rivers’ doctors allegedly did not follow the proper procedure when they sedated her with propofol at Yorkville Endoscopy in New York City, TMZ reported Tuesday. Officials said they believe the staff did not weigh the comedian before they administered the drug -- an egregious error since the amount of sedative is based on a person's bodyweight.

The clinic failed "to ensure that patient care services are provided in a manner that protects the health and safety of all patients,” the New York Department of Health and Human Services said, according to NBC News.

Rivers, 81, was ultimately given twice the amount of Propofol, the computer at the clinic showed. The staff member who oversaw the drug reportedly told the Health Department she made a mistake with the dosage and that gave Rivers double the amount.

That wasn’t the only procedure flaw, however. The "Fashion Police" star's personal doctor, Gwen Korovin, said, “I will go first.” Meaning, she would perform the first procedure. Korovin reportedly was in the operating room without authorization and did a laryngoscopy without Rivers’ consent. She had only signed-off on an upper endoscopy procedure, TMZ wrote.

Doctors were oblivious as Rivers lay dying on the table, the news site said. Her oxygen levels plummeted and she ultimately suffered brain damage.

River’s daughter Melissa Rivers is “outraged by the misconduct and mismanagement," a statement by her lawyers read. “Moving forward, Ms. Rivers will direct her efforts towards ensuring that what happened to her mother will not occur again with any other patient."

However, in a statement Monday, Yorkville Endscopy said the clinic remains open and is still fully accredited. "Yorkville has been fully cooperative and collaborative with all regulatory and accreditation agencies. In response to the statement of deficiencies, Yorkville immediately submitted and implemented a plan of correction that addressed all issues raised,” it read. “Yorkville will continue its commitment to complying with all standards and accreditation requirements."
