A Midtown Manhattan elevator accident killed a woman and injured two others on Wednesday, according to a report.

The New York Post reported that a woman was pinned between two floors while riding the elevator and crushed to death. The Post reports that the woman had taken a step halfway inside the car, but the elevator began to ascend with its doors open before she was fully in and pinned her between the two floors.

Two other people were injured, according to the FDNY, but the injuries are not believed to be serious. It is unknown what caused the elevator to malfunction.

The tragic death occurred at approximately 10 a.m. at 285 Madison Avenue near 40th street. The 25-story building is the home to Young & Rubicam Brands, one of the largest consumer advertising agencies.

It is unclear whether the unidentified woman worked for Young & Rubicam Brands.
