Derek (Patrick Dempsey) may be alive but he’s not out of the woods. According to a sneak peek video from the upcoming “Grey’s Anatomy” installment, titled “How to Save a Life,” everyone’s favorite doctor will be pulling out all the stops as he tries to save the victims of a fiery car crash with nothing but a teeny tiny first aid kit.

“You’ve got to be kidding me,” Derek says after learning he has only a small roll of gauze, medical tape and a few antiseptic wipes.

But leave it to McDreamy to figure out a way to use the minimal supplies he had in the back of his car to save the lives off all those injured in the accident.

So, how exactly does Derek find himself in the middle of a car wreck? Well, on his way to Washington, D.C. for a meeting, Meredith’s (Ellen Pompeo) husband gets cut off by a flashy sports cars.

“I’m doing 10 miles over the speed limit. That’s not fast enough for you?” Derek says in the episode 21 clip as the other driver swerves around him. The doctor, who was in a rush to get to the airport, then witnesses the reckless driver do the same thing to another car. However, the outcome ends in disaster when both vehicles flip over. Derek slams on his breaks as the two cars tumble down the road.

A preview from the upcoming installment shows that Derek manages to pull out a teen male from the wreckage who is bleeding profusely from his head.

“What’s wrong with him? Why isn’t he saying anything?” the female friend of the boy questions Derek.

“He hit his head really hard,” the doctor replies, revealing the victim has sustained a serious neuro injury. (Thank goodness that’s Derek’s specialty!)

But the boy’s bleeding dome is the least of Derek’s problems when the little girl from the accident shouts that the sports car is on fire. Derek warns the stable passengers to get away from the blazing vehicle while he carries the injured party to safety. Just as they get a safe distance away from the burning automobile, it explodes.

Looks like this episode will be heated with drama!
