A Silver Alert was issued Sunday for a North Carolina teenager who went missing when she left for a cookout. The girl disappeared after leaving her home in Winston-Salem.

Jayani Samarie Cross was last seen at 4344 Paula Drive. She is described as black with black hair and brown eyes. She stands 5 feet 4 inches tall and weighs 160 pounds and was last seen wearing a black T-shirt, a gray zip-up jacket, jean shorts, and black and orange Vans shoes. Authorities said that she could be in either a blue Kia sedan or a light-colored SUV.

Authorities said that the girl has been diagnosed with a cognitive condition. Other details about her health condition have not been revealed.
