Shonda Rhimes, creator of the hit ABC series “Grey’s Anatomy,” really knows how to pull at the heartstrings of viewers. When she’s not terrifying them with gruesome medical mysteries, she’s creating havoc in the personal lives of our Grey Sloan friends that's so intense it sends fans to bed with knots in their stomachs. Meredith (Ellen Pompeo) and Derek (Patrick Dempsey) have had a disproportionate share of the hardships in Season 11, but now it’s time for another couple to feel the wrath of Rhimes: Callie (Sara Ramirez) and Arizona (Jessica Capshaw).

Calzona’s relationship has survived a lot -- from Africa, to cheating scandals to car accidents. But now it looks like the duo will face a problem so big they might not be able to bounce back. Episode 5, “Bend & Break,” will focus on that storyline in the Callie/Arizona-centric episode of “Grey’s.”

“Everything leading up to this point has been a series of Band-Aids,” Ramirez told Entertainment Weekly. “Callie and Arizona are basically not communicating. Callie has some resistance toward therapy, but therapy turns out to be a great place to air grievances and concerns and to also bring up the miscommunication that has led them up to this point. They definitely get a chance to clear up some stuff.”

In a sneak-peek promo video for episode 5, fans watched an intense scene between Arizona and Callie during their therapy session. It begins with Arizona explaining how she went to Africa alone (for an opportunity of a lifetime) and when she came back (for Callie), her wife was pregnant. Callie brings the therapist up to speed explaining that she cheated on Arizona with a man. This leads Callie down a dark road, coming to the conclusion that Arizona feels “trapped” with a child she “never wanted.”

"Don’t you ever say that I don’t love my child or that I regret my child," Arizona fires back. "I regret that you made a decision for me. You make all the choices. And every time that I try to make a decision for myself, it’s like you make me feel like the worst, most selfish person in the whole world."

“I’m sorry. Did I make you feel selfish when you decided to sleep with another woman?” Callie quickly rebuts, concluding the trailer.

According to reports, Calzona’s therapist will try to mend their relationship by suggesting a treatment so shocking it could ultimately tear to two apart. “She thinks it’s crazy,” Ramirez said of her character’s reaction to the surprising plan. “She has a really hard time with it.”

What do you think the treatment could be? Well, whatever it is, it will have Callie running into the arms of Meredith and a bottle of tequila. “Meredith is going through her dysfunction in her relationship with Derek,” Ramirez explained. “They bond over having issues with their partners. And they also end up working together a lot. They actually end up working on a case where a man beats cancer, is in remission, then the cancer comes back and the cancer attacks his bones. They’re working on this case, and in the process, Meredith is also encouraging Callie to go to the bar and let loose a little.”
