You never know what to expect on “Survivor.”

Following the conclusion of Tuesday’s Season 32 installment, CBS aired a promo video for next week’s episode 4. While the network isn’t giving any hints about who will be headed home on the March 9 episode of the “Brawn vs Brain vs Beauty,” the preview for “Signed, Sealed and Delivered” teases one castaway may be leaving the game due to a medical emergency.

The teaser shows the contestants covered in dirt and digging holes in the sand.“I can see the exhaustion,” host Jeff Probst comments. The narrator goes on shares that the “most punishing season” of the series yet will continue “to take its toll.”

While all of the castaways appear tired by the task, Probst calls for the medical team to care for one participant. “We need water, water, water!” Probst yells. Medics are shown giving an IV to one player, though their face is not shown.

Probst then reveals the individual is going to need an evacuation and calls for a helicopter. Tai, who can be seen sitting next to Probst during the medical emergency, looks upset by the revelation. Could one of the Beauty tribe members be sent home?

Probst previously teased that “Kaoh Rong” episode 4 will be different than what viewers are accustomed to. “The fourth episode is epic. It's pretty full-on, and it's exciting because we tell the story in a new way,” Probst told TV Guide in February, revealing at least one contestant will quit for a medical reason.

So far, three castaways have been voted off “Survivor: Kaoh Rong.” On Tuesday, the first member of the Brain tribe was voted off. In her confessional Liz said she was shocked by her tribe’s decision. “I give them credit for playing a very good game and I hope one of them goes on to win,” she said.
