"The Good Doctor" Season 5 brought a shocking twist when it premiered Monday.

Season 5's premiere episode featured a new character, Salen Morrison (Rachel Bay Jones), an inquisitive patient who had a mysterious mass on her kidney that she avoided letting the team check out. She wandered around the hospital, asking questions about staff and workflow and interrogating attendants and chiefs of surgery.

However, it was eventually revealed that she was only testing the doctors and was already aware of her condition, scleroderma. She apparently wanted to see if the doctors at St. Bonaventure would be able to diagnose her condition quicker than her original doctors. Andrews (Hill Harper) passed the test.

A major plot twist came in the very last scene when it was revealed that Salen had purchased the hospital. Her questions about the staff and their workflow apparently had a specific purpose as she was now going to be their new boss, according to TV Guide.

Salen is expected to shake things up at St. Bonaventure. Her arrival will likely affect Shaun (Freddie Highmore).

"This tests everybody, including Shaun," executive producer David Shore previously told TVLine. "Of course, Shaun is going to have a different point of view on it."

The conflict was teased in "The Good Doctor" Season 5, episode 2 promo, in which Salen told the doctors, "We do not treat patients, we serve clients."

The doctors felt that following the changes, their jobs no longer depends on saving lives but on how their patients rate them through the smiley faces they will punch on a machine.

In one scene, Salen joined what appeared to have been a doctors' meeting, saying, "Don't let me interrupt." Shaun then responded, "You already have."

Salen then stared at Shaun before asking, "Are you on the autism spectrum?"

Another scene showed an angry and frustrated Shaun being comforted by his fiancée Leah (Paige Spara). "Change is something tricky for you," she tells him.

Meanwhile, Glassman (Richard Schiff) sat down with Shaun and told him he is thinking of moving on.

"I think I need to be around different people," Glassman said, to which Shaun replied, "Different from me?"

Glassman will likely have the most direct contact with Salen because he is the president of the hospital. However, their first encounter didn't go well. Glassman was not courteous to Salen when she visited his office for the first time and pretended to be a patient. If the tension doesn't ease between Salen and Glassman, the latter may not have a happy ending.
